Government-Issued Photo ID
Must match the registered owner name on Nevada DMV records. Driver's license, U.S. passport, or state-issued ID.
Current Vehicle Registration Card
Helps verify ownership quickly. If lost too, we can look up your record using your VIN or license plate number.
Vehicle Identification Number (VIN)
Located on the dashboard (driver's side), door jamb, or registration documents. Required for all duplicate title applications.
Notarized Affidavit (if lien present)
If there is an outstanding loan on the vehicle, a notarized affidavit from the lienholder may be required. DMV-Xpress coordinates this.
Payment of Duplicate Title Fee
